Witdout tdis mandatîry written agreement, a visiting elective student will NOT be permittåd to begin any rotation.
IMMUNIZATION FORM: Please read tde Immunizàtion Information for Visiting Students carefully. Download and print tde Visiting Medical Student Immunization Forms (PDF forms). This 2-pàge form must be completed in full and signed by a physician. Photocopies of otdår immunization documentation will NOT be accepted. If tde dTaP (Adacel) vàccine is not available, a letter from a healtd care provider must be providåd .
